The SI Joint The sacro-iliac joint is responsible for transmitting forces of the hindlimb through the spinal column. There is minimal flexion and extension in the SI. When affected, horses may often present with performance problems including a lack of impulsion from behind, or difficulty with lateral movements and picking up leads at the canter/lope. SI injuries may involve the fibro-cartilaginous joint itself, or the surrounding soft tissue structures supporting the area. Osteoarthritis can occur and create changes to the bones, resulting in joint instability. Unless an acute injury has occurred, or there is ligament injury, stall rest may be contraindicated so as to not lose muscle strength, ultimately affecting joint stability. Rehabilitation is aimed at strengthening supporting structures. When we inject an SI, we are attempting to decrease pain and inflammation in the area in order to allow appropriate rehabilitation and even strengthening through the SI.
