Decree of Entitlement or Adjudication from Legal Service For INP Transferee a. Retirement order (GSIS retirement vouchers) (PD 1184) b. Latest Certificate of GSIS Monthly Pension c. NSO Marriage Contract with receipt d. Original NSO CENOMAR with receipt (husband and wife) e. NSO Birth Certificate with receipt f. Update and ID Application Form (2pcs) g. 2x2 ID picture
