Tobias Rent had a quiet day. The early morning scare of tension pneumothorax was followed by a chest tube which eliminated the crushing chest pain. Tricky times with b/p dropping and requiring small doses of Levophed to keep the pressure up. 0.1to 0.8 mcg/kg up and down throughout the day. b/p now about 100/50 with 70MAP. Heavy sedation of Propofol and still on 10 of fentanyl for pain. This morning the only pain he complained of was the ET tube and I discussed a trach with him. He was in favor because of the throat pain. The doctor came by and we talked it over, he wants to get a consult tomorrow, friday, and then see how the weekend goes and put it in on Monday or tuesday. My thought is to go ahead and do it now when he is in a relatively stable condition and then if he is able to be weaned in the next 5-6 days, great, and let it heal. Obviously not in my control, so we will wait and see what happens tomorrow. Urine output has slowed a small amount so dialysis will repeat in the a.m. Big complaint of how hot his legs get with the compression devices. So I take them off twice a day, rub his legs with lotion and then put them back on. Suctioning gave great results sometimes and at others it was very small amount obtained. Marie and Karl signed their closing documents on their new home today. I sent the sellers agent flowers as she has been so compassionate, helpful and understanding during this difficult time.
