Job Description # Doing Direct upload Coding and Verification of Coded Batches as required. # Responsibilities include downloading the transcribed physician reports, assign proper ICD and CPT codes. Responsible for abstracting (ICD-9) (CPT-4) data from medical reports and encrypted forms. # Reviewing, organizing, summarizing, and coding patient medical records with the appropriate diagnostic and procedural codes. # Sorting of Medical records by facility, date, diagnosis, page number, etc. # Review of Medical history and capture of all demographics of the patient. # Page by page review of all medical records and assign appropriate ICD and CPC codes.
